Heart Failure Program

Maple View Health and Rehabilitation Center is located at 856 Maple Street in Rocky Hill, CT.  The Heart Failure program at Maple View provides in-patient sub-acute rehabilitation focusing on stabilization of acute and chronic heart failure conditions.


The program consists of an individualized, interdisciplinary treatment model.  The team includes the following:

  • Physician
  • Registered Nurses 7 days per week, 24 hours per day.
  • Therapy Services 7 days per week as needed.
    • Physical Therapy
    • Occupational Therapy
    • Speech Therapy
    • Respiratory Therapy
  • Registered Dietitian
  • Social Worker


A registered nurse performs a cardiac assessment every four hours for the first 24 hours of a patient’s stay.  After the first 24 hours assessments are performed each shift (3 times per day) until the patient is discharged home.  Weights and vital signs are monitored daily throughout the entire length of stay.  Our on-site clinical offerings are:

  • X-Ray Services (Chest X-Rays)
  • EKG’s
  • Laboratory services
  • Pulse Oximetry Testing
  • C-Pap and Bi-Pap
  • AED
  • Waiver to perform IV Lasix push if needed

All patients admitted to the Heart Failure Program will be evaluated by the appropriate rehabilitation discipline and will have the following assessments completed on admission, weekly and upon discharge:

  • Borg Dyspnea scale
  • 6 minute walking test (3 minute test if unable to tolerate 6 at first – will work up to 6 minutes)
  • 30 Second Sit to Stand test
  • Quality of Life Evaluation

Our Rehabilitation department provides individualized treatment plans to fit each patients need.  We utilize state of the art Accelerated Care Plus equipment to ensure the most up to date treatment options.  Some of our therapy equipment utilized in our program is:

  • OmniCycle
  • Omnistim
  • Proform Bicycle
  • Therapy Balls
  • Weights


Maple View provides one to one education with each patient through out the entire rehabilitation stay.  Our educational offerings include:

  • Heart Healthy Diet – by a Registered Dietician
  • Fluid Restrictions – how to manage at home
  • Reading food labels
  • Energy conservation techniques for day to day activities
  • Overview of their cardiac diagnosis
  • Medication management
  • Signs and symptoms of heart failure – Green–Yellow–Red Zones
  • Weight Monitoring
  • Telemonitor
  • Identification of negative past practices and supports that can be put in place
  • Stress reduction techniques
  • Purse lip breathing techniques
  • Compensatory ADL strategies

Our goal is to work closely together to decrease costly re-hospitalizations by providing education and treatment in a post-acute setting and evaluating the many intricacies of CHF including underlying causes, medications, and necessary lifestyle modifications.
